Has anyone setup Ansible Tower with a failover cluster?

yes but, is not a supported configuration and its going to change with how AAP is now. You can look at the code consulting created https://github.com/redhat-cop/automate-tower-ha-dr and see if it works with the version of tower you have. That is the best code that will work.
